Job overview An exciting opportunity has arisen for a self-motivated individual to join our small, friendly, professional and supportive Anticoagulation Team based at St Peter’s Hospital. We run a challenging and demanding yet rewarding anticoagulant service with a patient caseload of approximately 900 patients. In this role you will gain experience in managing all aspects of patient anticoagulation treatment and monitoring within a well defined dosage competency framework. The successful applicant will be working within a team as well as under their own initiative following specialist training. There are exciting opportunities to develop and extend the anticoagulation service. This is a part time role, 30 hours working 5 days per week, Monday to Friday. It would be an advantage for the successful applicant to have experience in managing patients on an anticoagulant, with a knowledge of warfarin and its pharmacokinetics. Julie Hargreaves Biomedical Scientist Julia Roper Anticoagulant Clinical Nurse Specialist Email: [email protected] Telephone : 01932 72 2161 or 01932 72 6426 Advert The post holder will work closely with clinicians, specialist nurses, patients, patients carers, district nurse (DN) teams to actively manage all aspects of a patients anticoagulation within the scope of the dosage competency framework. Internally the post holder will develop effective working relationships with: all service sites including Surgery and Medicine, Central Booking Service, patients and patients carers. Externally this includes: other NHS Trusts / GPs/ DNs The post holder will be responsible for the management of the patient’s anticoagulant pathway, from referral, through to initial patient education, treatment implementation and management via warfarin dosage advice. Liaising with other health care providers to ensure patient centred safe anticoagulation management. The post holder will provide warfarin dosage management for clinically stable patients, including patients on concurrent medications, patients with acute co morbidities, while being responsive to patients needs. Within the dosage competency framework. The post holder will provide initial warfarin or DOAC education as appropriate following national guidance. Then subsequent ongoing patients specific education for the duration of a patients warfarin The post holder will act as the patient advocate and liaise directly with the patients and their carers at all points along the anticoagulation therapy pathway. Keeping the GP appraised of all warfarin dosage advice.
