We have an exciting opportunity for a qualified Band 6 Social Worker to join our dynamic multi-disciplinary Specialist Mental Health Service for Older Adults, serving the needs of older people within East Hertfordshire, covering Broxbourne Borough, East Herts District, and Welwyn Hatfield Borough.
You will deliver care to an allocated caseload of service users experiencing or recovering from mental health difficulties. Responsibilities include conducting holistic assessments, developing support plans, reviews, and CPA in collaboration with other professionals, while building networks, raising awareness, and developing links for the team.
You will work alongside Consultant Psychiatrists, Psychologists, Nurses, Occupational Therapists, and Support Workers to achieve optimal outcomes. You will provide direct support and advice to service users, their families, and carers, within a recovery framework that emphasizes individual choice and control.

This role offers a Golden Hello of £3,500, payable as £1,500 on commencement and £2,000 after one year of employment.

Take a lead in social care, understanding legislation and promoting adult safeguarding. Conduct comprehensive assessments, especially for individuals with high risk or complex needs, ensuring appropriate care planning and safeguarding.
Support service users and carers through information, advice, and signposting. Ensure health and social care services are appropriately commissioned, including direct payments and charging processes.
Regularly assess risks and ensure the safety of vulnerable individuals. Conduct carer assessments and develop contingency plans.
