Base pay range
$77,000.00/yr - $122,200.00/yr

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
* Assesses patient’s health problems and needs and develops plan of care.
* Provides direct skilled nursing care as per physician orders.
* Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
* Document medical records and prepare other reports as needed.
* Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
Supervisory Responsibilities
* Completes home health aide supervisions and orientations as needed.
* Co-signs orders and provides oversight of care for LPNs.
QUALIFICATIONS:
* Graduate of an accredited school of nursing – required.
* Current license to practice as a registered nurse in the state of Massachusetts – required.
* Current CPR – required.
* One year nursing experience, preferably med-surg - preferred
* Possess a valid State of Residence Driver’s license and reliable transportation – required.
