Community Support Worker – Specialist Community Forensic Team
In this role you will work in Cheshire and Merseyside, liaising with partner agencies to develop risk mitigation and support for autistic and learning disabled service users.
Responsibilities
* Carry out assigned duties involving direct care and observation of service users under guidance and indirect supervision of qualified staff.
* Report observations to qualified staff, providing details of mental state, risk factors, and general observations.
* Work with service users on an individual and group basis as outlined in the care plan.
* Coordinate assessment and delivery of a planned programme of care with guidance from a qualified practitioner and contribute to formal assessment.
* Actively engage service users in the implementation of treatment and care plans, maintaining and developing social, self‑help, occupational and personal skills.
* Assist with activities of daily living, social, recreational and occupational activities as agreed in the care plan.
* Function as a key worker under the direction of the SCFT Lead Clinician and senior staff.
* Provide verbal and written feedback to a qualified practitioner, including concerns about physical or mental health.
* Contribute to the effective running of the service by providing and receiving information to/from other departments or services.
* Use verbal and non‑verbal skills to overcome communication barriers presented by service users.
* Maintain a safe environment for service users in the community.
* Carry out duties contributing to assessment of service users as requested by a qualified practitioner.
* Develop and maintain links with other agencies, statutory and voluntary, and support social care staff in delivery of appropriate care.
* De‑escalate situations where service users become physically or verbally aggressive.
* Work directly with service users in their own homes to complete assessments, deliver interventions and respond to crises where additional intensive support is required.
* Support delivery of group interventions and facilitate programmes with senior members of staff.
* Support training development for staff teams and groups under indirect supervision of senior staff.
* Maintain accurate charts and data collection protocols.
* Contribute to the development of risk management plans.
Working Hours & Contract
* Permanent position, rostered over a 7‑day week, 08:00–20:00.
* Full time – 37.5 hours per week.
