Clinical Pharmacist Primary Care

The Clinical Pharmacist is an integral member of the general practice multidisciplinary team, responsible for optimising medicines use, improving patient safety and health outcomes through expert medicines management. The role focuses on delivering safe, effective, and evidence-based care to a diverse patient population.
Main duties of the job
Conduct medication reviews, including structured medication reviews for patients with long-term conditions, complex polypharmacy, or high-risk medicines.
Support the management of chronic conditions, ensuring medicines are prescribed in line with national and local guidelines.
Provide clinical advice to GPs, nurses, and other healthcare professionals on safe and effective prescribing.
Lead on medicines reconciliation following hospital discharge and minimise medicines‑related incidents.
Engage in audits, quality improvement projects, and contribute to meeting local prescribing targets.
Support patient understanding of their treatment through counselling and education.
Identify and implement strategies for de‑prescribing and optimising cost‑effective use of medicines.
Assist with prescribing queries and patient requests.
Collaborate with community pharmacies and secondary care to ensure seamless patient care.

Person Specification
Qualifications
 * Registration with the General Pharmaceutical Council.
 * Experience of working in a clinical or primary care setting.
 * Strong knowledge of therapeutics, clinical guidelines, and medicines legislation.
 * Excellent communication, problem‑solving, and patient‑focused skills.
 * Experience conducting face‑to‑face and remote patient consultations.
 * Experience with clinical systems used in primary care (e.g., EMIS, SystmOne).
 * Independent Prescriber qualification or willingness to work towards it.
 * Knowledge of local prescribing initiatives and NHS pathways.
