Children Community Learning Disability Nurse
We are looking for 2 highly motivated Registered Learning Disability Nurses to join the Children and Young Peoples Learning Disability Service in Surrey. The role covers a county-wide service across 4 bases: Redhill, Leatherhead, Godalming & Chertsey.
CLD is a nurse-led service with multidisciplinary teams including psychologists, APs, psychiatrists, music therapists, nurses, and admin support. The role focuses on collaborating with families, carers, and young people to establish and manage care plans. The majority of needs are behaviour and anxiety related, and the service follows a PBS-based Behaviour Pathway.
You will hold a caseload of children and young people in the community, following a Discovery Conversation where the presenting need has been identified. You will demonstrate expertise and specialist nursing skills in areas such as crisis intervention, alternative communication, assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and other conditions. You will actively involve people in developing and agreeing their own care plan, act as an advocate and ensure that proper regard is given to their dignity, choice, self‑esteem, racial, cultural, sexual and religious needs and preferences. You will practice within the Positive Behaviour Support framework.
Responsibilities include:
* Developing and implementing nursing interventions within a holistic, person‑centred approach across various settings.
* Conducting comprehensive assessments and monitoring progress.
* Collaborating with education, school nursing, social care, paediatrics and other agencies.
* Attending and contributing to clinical issue meetings, business meetings and quarterly LD forums.
* Providing regular supervision and appraisal, and seizing opportunities for career development.
Qualifications:
* Registered Learning Disability Nurse.
* Knowledge of Positive Behaviour Support (PBS) and the I Thrive Model.
* Experience working with children is preferred, but experience with adults is also transferable.
* Valid UK driving licence and access to a car for travel across Surrey.
