Your workstyle will be a mobile hybrid worker in this role which gives you the flexibility to work from home and from a variety of Council workplaces.
About the role:
We are currently seeking to recruit a motivated and enthusiastic qualified Social Worker to join our team. You will work in partnership with adults who have varied needs, their carers and families to identify what matters to them, devise care and support plans to meet their outcomes, whilst maximising independence where possible. The focus of the teams is to build on the assets of individuals within their own communities and work with colleagues to identify this where appropriate.
We work alongside a range of professionals within the teams including colleagues from the health board, therapies and children’s services.
About you:
Applicants need to be qualified Social Workers and professionally registered as a Social Worker with Social Care Wales.
* Experience in Community Social Work within Adult Social Care.
* Experience in assessing and identifying outcomes and needs of individuals.
* Experience in designing and implementing formal care and support plans.
* Knowledge and experience of legislation and policies relating to adult social care, such as the Social Service Wellbeing (Wales) Act (2014), Mental Capacity Act (2005) including Deprivation of Liberty Safeguards, Continuing Health Care and Positive Risk Taking.
