The aim of Social Work Resources is to promote social welfare and provide effective care and support to meet the needs of vulnerable people in South Lanarkshire.
In partnership with community planning partners, voluntary and independent provider organisations, we offer a range of services designed to enable, support, improve, and protect the health and social care of people using our services.
As one of the largest services provided by the Council and the South Lanarkshire Health and Social Care Partnership, the Care at Home service plays a critical role in enabling individuals to remain at home in a way that promotes their wellbeing, dignity, privacy, respect, and personal choices.
South Lanarkshire Health and Social Care Partnership, alongside its commissioned Care and Home partners, provides Care at Home services to over 3,000 service users daily.
The service is continuing an exciting programme of transformation. To support this work, we are looking to appoint Social Work Assistants. As part of the Care at Home team, you will be responsible for ensuring the Council meets its statutory obligations, conducting timely assessments and reviews of service users to mitigate risks and ensure their health and wellbeing needs are met with appropriate services.
