Mental Health / Homeless Nurse – County Durham


Overview

We’re seeking a dedicated Mental Health / Homeless Nurse to join an outreach?focused service supporting individuals with complex needs across County Durham.

This role is ideal for an experienced RMN confident in working autonomously, providing specialist support to people who are:

You will contribute significantly to the dual-diagnosis pathway, liaise with partner agencies, and take clinical governance responsibility within outreach services.


Key Responsibilities


Clinical Duties

* Manage a complex caseload independently.
* Conduct mental health and risk assessments.
* Provide general health interventions and prescribing assessments.
* Deliver safe, evidence?based prescribing aligned with current guidelines.
* Provide health promotion and harm?reduction support.
* Advocate for service users and promote recovery?focused care.


Outreach & Engagement

* Provide direct outreach support to rough sleepers and hard?to?engage individuals.
* Work closely with harm?reduction and rough?sleeping outreach teams.
* Build trusting therapeutic relationships and support engagement in treatment.


Governance & Leadership

* Lead mental health governance within outreach services.
* Monitor clinical standards and support quality improvement initiatives.
* Provide clinical supervision to colleagues.
* Support CPD, practice education, and guideline implementation.


Partnership Working

* Collaborate with internal teams and external agencies.
* Work alongside substance?misuse workers, mental health services, primary care, and social care.
* Contribute to integrated dual?diagnosis pathways.

Essential Criteria

* Registered Mental Health Nurse (RMN)
* Experience supporting clients with substance misuse
* Understanding of the social context of homelessness and substance misuse
* Experience delivering assessments and safe prescribing practice
* Competency in general health assessments and harm?reduction delivery
* Experience providing clinical supervision
* Strong knowledge of current prescribing guidelines
* Understanding of recovery?oriented primary care treatment models
* Advocacy skills and a solution?focused approach


Compliance Required

* CV with relevant clinical experience
* Date of Birth
* Right to Work documentation
* NMC PIN
* Up?to?date ILS
* Enhanced DBS (prison clearance not required)
