About the Role
We are looking for a motivated and compassionate Support Worker to join our accommodation-based service in Oxfordshire. This service supports individuals who are either transitioning from hospital into the community or at risk of hospital admission, offering a safe environment where their needs can be assessed and supported.
The service plays a vital role in helping people with complex health and social needs stabilise, recover, and move towards independent living.
Key Responsibilities
Provide high-quality, person-centred support to residents with complex needs
Carry out needs assessments and develop tailored support and move-on plans
Manage a caseload of clients, ensuring regular reviews and progress tracking
Work collaboratively with healthcare professionals, housing providers, and other agencies to deliver holistic support
Promote independence, wellbeing, and positive health outcomes for residents
Maintain accurate records and contribute to a safe, supportive living environment
About You
Experience working with individuals with complex or multiple needs ( homelessness, mental health, physical health)
Strong communication and interpersonal skills
Ability to work both independently and as part of a small, dynamic team
A proactive and flexible approach to problem-solving
Understanding of safeguarding and professional boundaries
