Our health partners are creating a community intervention offer with us in a multi-disciplinary context. We are developing pathways to support safe hospital discharge and to provide wrap-around care with primary care networks to help people stay safe, supported at home, and remain in their community at end of life.

The Roles
We have opportunities for passionate, committed Social Workers in our multi-disciplinary teams across Cornwall in both full and part-time roles. This is a public/customer-facing role with the statutory English language requirement for public sector workers. The position will be subject to an enhanced criminal record disclosure check. Full time - 37 hours per week.
What You’ll Need To Succeed
We welcome applications from newly qualified Social Workers (NQSW) who need to complete their first Assessed and Supported Year in Employment (ASYE). Please refer to the Grade 3 Social Worker (Adults) role profile when applying.
You will be a Social Worker qualified with a Social Work Degree, Dip. SW, or equivalent and registered with Social Work England, who will have:
* Expert knowledge and experience of relevant legislation, social work theory, policy and principles, with evidence in practice.
* Previous experience in a social care environment with a proven track record of strengths-based practice.
* Ability to undertake, analyse and translate complex assessments of need, risks and strengths.
* Proficiency in recognising and responding to behaviour that may indicate resistance to change, ambivalence or selective cooperation with services, and the ability to deliver difficult messages when needed.
* Ability to form strong professional relationships with other partners within a multi-disciplinary setting.
* A high level of written and verbal communication skills.

Please note: If you qualified as a Social Worker within the last 2 years but have not completed ASYE, we welcome your application as a Newly Qualified Social Worker (NQSW) to access Cornwall’s ASYE programme.
