Transition Support Worker [Tameside Met BC] Pay rate £19.99 per hour Vitalis are recruiting for a Transition Support Worker to work in a Staying Close Project in Tameside. Tameside Children's Services are seeking to recruit committed Transition Support Workers to join our team as part of the Staying Close project. Staying Close is a pilot service offer to children who have left care and who have previously lived in children's homes, designed to offer them practical, employment, emotional and social help in their lives as they exit care. The aim of the programme is to ensure that each young person is equipped with skills to be able to live a successful and positive life, once they move into their own home. The support to the young people continues until the age of 21.
