Cancer Care Co-ordinator

South Tyneside and Sunderland NHS Foundation Trust have anexciting opportunity for a highly motivated, compassionate individual to joinour personalised care team within the Cancer Information and SupportCentre.

We are looking for a dedicated and enthusiastic individual toactively support cancer patients, their families, friends and carers. Thepostholder will manage referrals into the Cancer Information and Support Centreand support service users with their nonclinical support needs, providinggeneral advice and information on cancer, treatments and side effects, offeringpsychological and emotional support and practical support based on needs.

The successful applicant should have excellent communicationskills and be able to demonstrate efficient organisational and multitaskingskills with the ability to prioritise effectively and efficiently in afast-paced work environment.


Main duties of the job

Thesuccessful applicant will provide a first point of contact for cancer patients,their family, friends and carers, proactively managing telephone calls, emails,drop ins and referrals from clinical teams via trust systems. There will be astrong emphasis on patient information, including onward referral to servicesthat will meet patients needs.

Theywill demonstrate excellent organisational skills, must be flexible in approachand be able to work on own initiative whilst demonstrating a consistently highstandard of professionalism, being aware of the need to confidentiality andintegrity. Administrative abilities and experience of working with computerpackages such as Microsoft and Meditech are essential for this post

Job responsibilities

Provide effective, caring and compassionate services to patients working alongside multi-disciplinary teams (MDT) to deliver a high standard of care and support to patients with cancer. This will involve co-ordinating care and supporting patients and their families from referral to living with and beyond cancer. This will require regular contact with patients suffering from cancer. You will be expected to support and signpost patients face to face and over the telephone and ensure effective progress along their care pathway in liaison with other colleagues and departments in the hospital. The Cancer Care Coordinator will work collaboratively with cancer teams, providing non specialist planned interventions and support to improve cancer patient care and experience.

To assist the registered practitioner in providing high quality compassionate care and assistance to patients / relatives and their carers appropriate to their needs. Responsible for carrying out tasks as delegated by the registered practitioner who has overall accountability for delegating care.

Person Specification


Physical Skills

* Ability to assist patients to move, as per Trust Moving and Handling Policy
* Flexible and adaptable to change
* Self-motivated with the ability to engage and motivate others
* Able to cope under pressure


Qualifications

* NVQ Level 3 or above in Business Administration or Health and Social Care or equivalent relevant qualification
* GCSEs grade C /4, including Numeracy and Literacy and above or equivalent)
* Willingness to complete a Level 2 Healthcare Support Worker (Care Certificate) qualification on commencement to post (if not already achieved).
* Willing to undertake further training related to role.
* Willing to complete Competency Based Assessments and a Portfolio of evidence, in order to achieve the Care Certificate.


Experience

* Recent experience working with cancer patients or in cancer services.
* Previous NHS experience or equivalent
* Experience working with a multi-professional team.
* Experience of working with medical or clinical teams
* Experience of direct patient contact including telephone and face to face contact.
* Demonstrable experience in an administrative role.
* Experience of communicating with clinical and medical staff
* Experience of dealing with confidential and sensitive data and appropriate maintenance and storage of records.
* Previous experience of dealing with customers, patients and/or members of the public.
* Experience of managing resources.
* Experience of negotiating with and influencing other staff.
* Experience of Programme/Project support work
* Experience of using STSFT information systems.


Skills and Knowledge

* Ability to communicate messages and instructions accurately to colleagues, both verbally and in writing.
* Demonstrate good customer care skills and the ability to communicate effectively with the Multidisciplinary team, patients, carers and visitors.
* Basic skills of empathy and reassurance
* Keyboard and IT skills including knowledge of Microsoft Word and Excel
* Excellent communication skills (written, verbal and listening) with proven experience of dealing with a wide and complex range of stakeholders and matters.
* Be able to interpret and follow written instructions from members of the multi-disciplinary team.
* Ability to adapt to new working practices, learn new software systems and acquire new knowledge.
* Able to construct clear grammatically correct correspondence including letters, emails and reports.
* Able to prioritise and plan work in a busy environment and to tight deadlines when required.
* Ability to work under pressure with low levels of supervision.
* High level organisational and co-ordination skills.
* Ability to demonstrate integrity and discretion when working with confidential matters.
* Ability to work as a member of a team, closely and harmoniously with others.
* Administration support of a multi-professional team
* Have an awareness of Trust policies and procedures.
* Knowledge and understanding of the NHS in wider context.
* Insight into cancer services and related i.e. trained in Sage and Thyme or recognized Counselling certificate.
* Ability to understand cancer targets.
* Insight into roles and responsibilities of wider health and social care, community and voluntary sectors
* Evidence of Continuing Professional Development
