Mental Health Team - Social Workers (Mental Health Discharge)

Grade 10 (£41,511 - £44,711)

Are you an enthusiastic and experienced Mental Health Discharge Social Worker looking for a rewarding opportunity? Dudley is seeking two dedicated professionals to join our friendly and dynamic team. Successful candidates will work in both hospital and mental health settings. This is a vital role that involves preparing individuals for a safe and supportive transition back into their community after receiving mental health care.

As a key member of the multidisciplinary team, you will focus on citizen-centred and strengths-based approaches throughout the citizen’s journey, from pre-admission to discharge.


Responsibilities

* Assessment and Planning: Evaluate the individual\'s needs, including social circumstances, mental health requirements, and complex factors impacting their recovery. Collaborate with the multidisciplinary team to develop a comprehensive and timely discharge plan. Resolve disputes over support packages or specialist accommodation, particularly for individuals \"out of area\".
* Advocacy and Support: Empower individuals to make informed choices, understand their rights, and navigate mental health and social services complexities. Advocate for their needs to ensure they receive necessary community support.
* Connecting with Community Resources: Identify and connect individuals with appropriate community resources like housing, financial support, support groups, and mental health services. Link to commissioners\' and clinicians\' trust in supported housing.
* Family and Carer Involvement: Work with families and carers, providing information, support, and guidance to ensure everyone understands the discharge plan and how best to support the individual.
* Collaboration and Coordination: Collaborate effectively with other professionals, including psychiatrists, nurses, therapists, and community-based organisations, to ensure a smooth and coordinated discharge process. Address inconsistencies in recording reasons for delay.
* Mental Capacity and Safeguarding: Ensure individuals are protected from harm and their rights are respected by being knowledgeable about mental capacity and safeguarding issues.
* Addressing Complex Circumstances: Experienced in working with individuals facing complex social circumstances such as homelessness, poverty, or trauma.
* Mental Health Act: Familiar with the Mental Health Act and ensure individuals are discharged according to legal requirements. Undertake section 117 reviews under the Mental Health Act and work within the legal frameworks of the Care Act, Mental Capacity Act, Human Rights Act, Equality Act, and Social Work England requirements.
* A Culture of Risk Aversion: Address the reluctance to discharge citizens without care packages, ensuring they are not admitted without therapeutic need.
* Information and Data: Enable more robust data sharing across systems to understand reasons for delays.


Additional Information

There are opportunities for career progression and personal development, including training opportunities and access to peer support from a well-established team. You will also have the opportunity to complete practice educator, BIA, and/or AMHP qualifications.

Please note that these posts are 12-month rolling contracts, offering flexibility and adaptability to attract Social Workers who prefer this type of work.
