The PCN hasalready established excellent collaboration in delivering pharmacy provision tothe many aspects of the DES within its locality and this multi-site role will helpunderpin and support the continued development in an ever changing environment.

Main duties of the job
The post holder will work within their clinicalcompetence as part of a pharmacy team to provide expertise in clinicalmedicines management.
The post holder will perform medication reconciliationfrom clinic and discharge letters and level 1 medication reviews of patients.
The post holder will provide quality improvement andclinical audit and manage some aspects of the Quality and Outcomes Framework. Thisrole is pivotal to improving the quality of care and operational efficienciesso requires motivation and passion to deliver an excellent service withingeneral practice.

Four Counties PCN serves apopulation of approximately 39,600 residents across Bourne, Stamford and surroundingvillages. We have named the PCN Four Counties as we support residents acrossLincolnshire, Rutland, Northamptonshire, and Cambridgeshire.
Job responsibilities
Therole of pharmacy within General Practice is part of an exciting programme oftransformation which is underpinned by the NHS 10 year Plan.
Thisis an innovative role to work within the Lakeside Healthcare Pharmacy team toensure safe and effective use of medicines for patients within LakesideHealthcare.
As a starting point the role willinitially focus on medicines reconciliation from clinic and discharge letters,and will develop further as the role of the Pharmacy team expands within theorganisation. It is vital that effective systems and procedures are in place toensure that this process is effective, safe and efficient. The right candidate will be involved in dayto day management of this with regular and thorough reviews of all medications.The process also ensures that patients receive the appropriate monitoringnecessary for the medication they are prescribed.
Person Specification
Qualifications
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences
* Professional Registration with the GPhC
Experience
* Evidence of continued professional development
* Experience of working as a Pharmacy Technician in either community, ICB or hospital pharmacy
* Proven experience working as a Pharmacy Technician within Primary Care
