What you will do:
* Lead direct practice with families, conducting assessments and developing tailored intervention plans to support children with SEND.
* Utilise your experience to carry out assessments, care planning, and crisis intervention for children with disabilities.
* Implement early interventions and mobilise resources to prevent escalation of family needs.
* Coordinate multi-agency support, ensuring a holistic and child-centred approach to care planning.
* Apply your knowledge of safeguarding, legislation, and child protection frameworks to guide best practice.
* Deliver the graduated model approach, providing personalised and evolving support based on family needs.
* Support young people with SEND in preparing for adulthood, focusing on independence and life skills.
* Engage in reflective supervision and contribute to team learning and continuous improvement.
