Pontefract and Knottingley PCN is recruiting an experienced, enthusiastic, and forward-thinking Health and Wellbeing Coach to join our established team, covering a population of over 49,000 across 4 practices in the area.
Health and Wellbeing Coaches work as a key part of the primary care network (PCN) multi-disciplinary team. They support individuals to proactively manage their health conditions based on what matters to them, considering the whole person, promoting lifestyle medicine approaches, and encouraging the proactive prevention of illnesses. The role involves using coaching skills to support those with lower levels of patient activation to develop knowledge, skills, and confidence in managing their health and wellbeing, while increasing their ability to access community support.
Main Duties of the Job
As an integral part of each practice within the PCN, Health and Wellbeing Coaches collaborate with clinical and non-clinical colleagues, management, and the wider PCN to ensure effective delivery of outcomes. They support patients in developing their knowledge, skills, and confidence to manage their health, using coaching techniques across multiple sessions to help patients identify needs, set goals, and achieve personalized health objectives.
The role may involve working with patients via phone, videoconference, or face-to-face. Responsibilities include supporting self-management education, peer support, caseload management, facilitating group consultations, and developing projects to promote active lifestyles that improve health outcomes, community safety, and independent living.

You will join an existing ARRS team comprising Health and Wellbeing Coaches, Care Coordinators, Pharmacists, a Mental Health Practitioner, an Occupational Therapist, and a Social Prescriber (currently Live Well Wakefield).

Person Specification
Experience
* Experience in health and nutrition work with patients/clients.
* Understanding of the NHS landscape, including relationships between practices, PCNs, and commissioners.
* Knowledge of the biopsychosocial model of health and social determinants of health.
* Experience with coaching approaches/frameworks or other helping strategies like Motivational Interviewing.
* Experience working with groups.
Qualifications
* Degree in Nutrition, Exercise, Health, or equivalent qualification.
* Willingness to attend a minimum 4-day training in non-clinical Supported Self-Management health coaching from a PCI-accredited trainer.
* Coaching/counselling qualification or relevant experience involving reflective listening skills and training in non-clinical SSM Health Coaching.
Skills & Knowledge
* Ability to apply health coaching in group settings.
* Competence in using tools and techniques like agenda setting, goal setting, and problem solving within a biopsychosocial framework.
* Skills in supporting behavior change.
* Knowledge of self-management evidence base and development in the UK.
* Understanding of helping people with long-term conditions develop confidence and skills for health management, with familiarity of available models and tools.

* Flexibility to work outside core hours.
* Satisfactory DBS check.
* Evidence of ongoing professional development.
* Own transport and ability to travel regularly within the locality.
Disclosure and Barring Service Check
This role is subject to the Rehabilitation of Offenders Act 1975 (Exceptions Order). A DBS check will be required prior to appointment.
Salary: £28,229 to £30,843 per year, depending on experience and skills.
