Overview
To provide high-quality administrative and clerical support to the nursing and medical staff on the AMU inpatient wards and the Medical Same Day Emergency Care (SDEC) department. The role includes responsibility for managing patient information and ensuring the accuracy of data quality. It also involves assisting nursing staff with discharge planning and providing a professional and welcoming front-of-house service for patients, visitors, and staff, ensuring they are appropriately informed.
Responsibilities
* Update and maintain accurate patient data, including complete patient demographics such as next-of-kin and ethnicity details, on the EPIC system and within medical records.
* Enter and retrieve data from all current Trust computer systems, ensuring adherence to acceptable data quality standards. Use circulated data quality reports to correct any omissions.
* Ensure that full medical records are available for all patients admitted to the ward by uploading paper notes to EPIC and digitising all paper documents in a timely manner.
* Support clinical staff with the smooth flow and timely discharge of patients.
* Provide a full front-of-house service by welcoming patients and their relatives/visitors onto the ward, while ensuring that security and infection control measures are strictly followed.
* Answer ward telephone enquiries promptly, efficiently, and politely. Resolve local issues where possible, or refer them to relevant staff as appropriate. Be mindful of the sensitivity of such calls, and ensure that all messages are relayed accurately via email or written notes.
* Coordinate the transfer of patients to tertiary centres or continuing care settings (e.g. nursing homes, residential homes, or hospices). This includes arranging transport and ensuring the clinical team completes all relevant documentation prior to discharge.
* The post holder has a general duty of care for their own health, safety, and wellbeing, as well as that of work colleagues, visitors, and patients within the hospital, in addition to any specific risk management or clinical governance accountabilities associated with this post.
* To observe the general responsibilities of all staff under the Health & Safety at Work Act.
* To maintain strict confidentiality of staff and patient records, in line with Trust Policy; any unauthorized breach of confidentiality will result in disciplinary action.
* To ensure that acceptable standards of data protection are maintained, in accordance with the Data Protection Act.
* To observe the rules, policies, and procedures at Kings College Hospital.
* To undertake any other duties appropriate to the grade which may be required.
* To carry out duties as an employee and service provider with due regard to the Trust's Equal Opportunities Policy.
* To participate in team meetings as required by your team leader.
