Responsible for the assessment, care, and support of patients and their families with life-limiting illnesses, attending for end-of-life care or symptom management.
Assess, plan, implement, and evaluate care within your scope of responsibility.
Assist in managing and organizing work in the clinical area.
Act in accordance with the NMC Code of Professional Conduct and Hospice policies and procedures.

Job responsibilities
Clinical Responsibilities
* Promote evidence-based best practice care.
* Assess, plan, implement, and evaluate clinical care; act as a named nurse for a patient group.
* Ensure adherence to Hospice policies and procedures.
* Participate in audits and practice development.
* Assist in medicines management, including controlled drugs, following policies and NMC standards.
* Carry out nursing procedures in line with guidelines.
* Maintain accurate electronic records per NMC standards.
* Complete risk assessments timely.
* Collaborate with the multidisciplinary team through meetings and ward rounds.
* Handle and store patient valuables securely.
* Follow admission and discharge policies.
* Support clinical governance and appropriate care delivery.
* Support induction of new staff.
* Order supplies efficiently.
* Maintain up-to-date clinical knowledge, especially in palliative care, and participate in the 24-hour advice line as per rota.
* Recognize deterioration in patients and escalate appropriately.
* Respond sensitively to patients with special needs or learning disabilities.
* Use professional judgment in patient assessments and involve senior staff as needed.
* Organize own and junior staff time effectively.
* Ensure treatments and procedures are delivered accurately.
* Provide cover for colleagues as directed.

* Teach and supervise students and support workers under supervision.
* Take charge of shifts and allocate work appropriately.

Person Specification
Skills
* Excellent communication skills, both written and verbal.

Qualifications
* RGN registration.
* Completed palliative care training or courses.
* Bereavement or counseling courses.
* Relevant palliative care qualifications.
