PRIMARY JOB FUNCTION

The post-holder will be part of a multi-disciplinary team (Social Workers, Occupational Therapists, and Physiotherapists) which provides a health and social care service to the local community. In general Occupational Therapists will:

* be expected to care coordinate a caseload of people with varied complexity of needs.

Occupational Therapists at the higher grade will be expected to support those at the lower grade with complex cases as directed by the Team Manager.

* The Occupational Therapist plays a lead role in coordinating the care and support for users (and their carers/ families) pulling in the expertise of other professionals as required.
* They will also be responsible for accurate and comprehensive inputting of case management information for monitoring and planning purposes.
* The post-holder will aim to provide an efficient and cost effective health and social care service to service users, carers and their families, in accordance with agreed threshold criteria and using appropriate resources of Social Services, Health, the Council, the community and other non-statutory organisations.
* Occupational Therapists will be expected to adhere to the Health and Care Professions Council (HCPC) standards of proficiency for Occupational Therapists.
* The postholder will be responsible for maintaining Continuous Professional Development (CPD) as required to maintain their registration.
* Islington has adopted The College of OT‘s Code of Continuing Professional Development for OTs and expect OTs to ensure they work to the required competencies which can be found on the website of the College.
* Occupational Therapists will be required to attend post qualification training as required by the service.
* The post-holder may be required to work flexibly over a 7 day working week to fit in with the needs of the service.


DUTIES AND RESPONSIBILITIES


PRINCIPAL ACCOUNTABILITIES

1. Customer Care
* To manage all clinical and professional aspects of complex casework, where appropriate, using evidence-based occupational therapy assessment, intervention, planning and reviewing/evaluation skills.
* To carry out all duties within the principles of Personalisation and enable people to retain their independence for as long as possible.
* To actively identify and maximise opportunities to re-enable service users to ensure that they continue as long as possible to live independent lives and maximise their choice and control.
* To manage cases effectively and respond to service users’ needs in a timely manner within a highly pressured environment.
1. Care Co-ordination and Joint Working
* Work in a collaborative way and seek advice of internal and external professionals e.g. OTs, Physiotherapists, District nurses, or other appropriate professionals to promote the wellbeing of the individual and ensure they have access to the required level of co-ordinated care.
* Offer advice and information regarding statutory and voluntary resources across the Authority to members of the multi-disciplinary team and other members of the social work team.
* Develop and promote the role of health and social work within the multi-disciplinary team.
* Where appropriate act as Care Coordinator to ensure a seamless service to individuals with complex needs and avoid unnecessary admissions to institutional care.
* Where appropriate refer on to other health, mental health or social care services e.g. Children and Families/District Nursing and work in partnership where this is required.
1. Financial Resource Management
* To ensure that individual budget allocations are commensurate to the assessed needs and that the appropriate approval processes are followed.
* To take responsibility for financial expenditure within the service e.g. oyster cards, travel permits and to ensure that protocols are adhered to.


GENERIC DUTIES AND RESPONSIBILITIES

* To carry a caseload of varied range of complexity.
* To take on care coordination responsibility for their caseload.
* To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005
* To ensure that the Care Act 2014 is implemented in all aspects of their role.
* To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.
* To obtain informed consent for all care coordinated cases.
* To ensure that their Senior Practitioner is kept informed of any significant events relating to the service.
* To provide advice and consultancy to colleagues within the immediate team as well and to a wider range of stakeholders, which may include the NHS, the Independent sector and members of the public as appropriate.
* To keep records of work undertaken and entering data on the database in accordance with Council Policy.
* To participate in an urgent response system as directed by the Team Manager.
* To promote the Personalisation agenda and support implementation of Self Directed Support
* To facilitate development and validation of support plans and record unmet need.
* To investigate and respond appropriately to complaints.
* To attend multi- disciplinary, locality and other meetings as required.
* To meet contractual agreement by attending regular supervision.
* To take responsibility for promoting and safeguarding the welfare of vulnerable adults in your care and those who you come into contact with.
* To undertake other duties commensurate to the grade of the post.
