Overview
The post holder will work closely with the Lead Pharmacist for Medical Division to provide a comprehensive ward-based clinical service to the Medical Division. The role includes taking medicines histories, completing medicines reconciliation, therapeutic drug monitoring, making therapeutic interventions within scope of practice, supply of medicines, final checking of medicines and counselling patients. Management responsibilities include line management of junior staff, assisting with the day-to-day running of the service and monitoring performance. The role will involve continuous service improvements, developing the technician role, and integrating fully in the multi-disciplinary team. There is operational commitment to the Pharmacy service on weekends and Bank Holidays on a formal rota (currently 1:6). (Rotation frequency subject to change)
Main duties
Clinical responsibilities:
* To work towards and complete the level 4 Clinical Diploma for Pharmacy Technicians.
* To ensure medicines reconciliation is completed in a timely manner by communicating with patients, carers and GPs to ensure an accurate record of medicines and resolve discrepancies or issues.
* To ensure patients\' medicines are available in a timely manner during inpatient stay by checking Patients\' Own Drugs and completing medication orders.
* To follow Trust procedures for special dosing regimens or medications and ensure nursing staff are aware of any specialist modifications where additional interventions may be required, e.g. Parkinson\'s medication, liaising with ward staff at handover.
* To ensure medicines are available for timely discharge, ensuring patients and carers have received adequate counselling on their medicines and any changes made during the inpatient stay.
* To complete adequate transfer of care information to Carers, GPs, Care Homes, Domiciliary Care teams, Community Pharmacies and communicate clearly where clinical follow up is required as stated in the electronic discharge record.
* To train and support pharmacy and ward staff to work to a high clinical standard, making patient safety and wellbeing a priority.

Education & Qualifications
* NVQ3 Pharmacy Technician/Registered with the GPhC as a Pharmacy Technician
* Accuracy Checking Technician Qualification
* Achieved Accredited MM/Medicines Optimisation qualification or willingness to complete if not already achieved
* Level 4 Clinical Diploma or willingness to complete if not already achieved
Skills, Knowledge & Abilities
* Communicate effectively to all levels of staff
* IT Literate in routine Microsoft applications (Word, PowerPoint, Excel etc)
* Extensive knowledge of clinical pharmacy and ward-based working
