Job Summary
Exciting opportunity for an experienced Stroke physio/OT/SLT/nurse with leadership experience to become team leader of the Walsall Combined ESD/Community Stroke Team and Community bed‑based service. This role is part of Walsall Healthcare NHS Trust and leads the team delivering against the Integrated Community Stroke Service (ICSS) clinical guidelines, covering ESD, Community stroke, bed‑based or nurse review pathways.
Successful applicants will have excellent communication and organisational skills, enthusiasm, and a passion for developing the team. They will lead the multidisciplinary team in delivering high standards of care measured by audit, outcome measures, and patient satisfaction surveys.
Key Responsibilities
* Provide clinical and management leadership for community stroke care services and manage a multidisciplinary community‑based team across a geographical area.
* Contribute to improving practice, development, monitoring, and evaluating stroke services, including participation in research, clinical audit and QI initiatives.
* Supervise, support, advise and educate patients, relatives, carers and colleagues across the healthcare community.
* Manage the clinical caseload, demonstrating evidence‑based practice in assessment, planning and implementation of care across the community stroke pathway.
* Develop, support and maintain good working relationships with internal and external staff, building links with primary and secondary care, local authorities and independent/voluntary sectors.
* Ensure staff are supported with development, education and training, including PDRS and 1‑1 reviews.
* Support the Clinical Team Leader in delivering the National Stroke Strategy, SSNAP and ICSS.
* Ensure the service budget is managed within allocation, monitoring sickness, absence, leave, and handling complaints in line with Trust policy.
* Maintain robust performance management data and evaluation systems for a patient‑focused, cost‑effective and efficient service.
Essential Qualifications
* Trained Nurse with NMC qualification or SLT, Physio, OT with HCPC registration.
* Recent postgraduate training.
* Degree‑level education.
Desirable Qualifications
* Masters qualification.
Essential Experience
* Previous experience working in an MDT team.
* Extensive stroke experience.
* Previous leadership experience.
* Experience of QI.
Desirable Experience
* Previous experience working in the community.
