Want to play an integral part in one of our community mental health hubs? Then we have the role for you! If you are passionate about making a positive impact on the mental health and well-being of individuals and families, we encourage you to apply to join our dedicated team as a Community Mental Health Practitioner. You'll be responsible for enhancing the care for service users with complex mental health needs by planning and implementing appropriate care using validated rating scales, risk profiles, assessment protocols, and individual care plans.


What you'll bring:

* Proven experience of delivering high-quality, 1:1 support in community settings.
* Commitment to supporting clients to take an active role in shaping the services they receive.
* Knowledge of trauma-informed, recovery-focused practice and a real commitment to inclusion and anti-racism.
* A full UK driving licence and access to a vehicle for work purposes.
