The Enhanced Care Worker (ECW) will provide a full range of therapeutic interactions using activity based interventions and provide support to patients on a one‑to‑one basis whilst maintaining respect for the individuality, confidentiality and dignity of the patient.

They will work collaboratively with the multi‑disciplinary team to ensure a high standard of one‑to‑one care is delivered following assessment and planning by an Enhanced Care Coordinator.

The post holder is an active member of the MDT, undertaking routine personal care activities for patients – bathing, toileting, dressing, supporting with meals and refreshments. The post holder has a personal responsibility and duty of care to patients/service users.

The post holder must comply fully with all Trust policies and protocols, including those related to patient safety and the prevention of healthcare‑associated infections. They must promote dignity, privacy, and respect, delivering compassionate, safe, high‑quality patient care.

As a Trust employee, they must honour work commitments, be reliable and trustworthy, uphold the Trust values and code of conduct, and display civility towards others.

The post holder will be expected to complete the Care Certificate within the first six months of employment and will participate in ongoing learning and career development.


Responsibilities

* Provide high quality and consistent enhanced care to patients with complex health and social needs.
* Deliver one‑to‑one Enhanced Care within an acute ward environment as directed by the Enhanced Care Coordinator, including observation of suitable patients (e.g., dementia, learning disability, mental health, drug & alcohol withdrawal, falls risk, abscond risk, self‑harm, delirium).
* Support patients with personal care – bathing, toileting, dressing, use of hoists, wheelchairs and other equipment to safely transfer or position patients.
* Assist with nursing and hygiene tasks such as routine nail and hair care, and basic continence needs.
* Assist with nutritional and hydration needs with staff permission and guidance, and record input and output where required.
* Accompany patients off the ward for appointments and transfers to care homes with staff permission.
* Engage patients in meaningful and therapeutic activities – dominoes, cards, drawing, jigsaws, etc. – using activity bags and technology such as RITA and the Magic 360 Table.
* Maintain accurate records – update basic charts and electronic or paper‑based systems after completing the Health Care Record Keeping course; entries must be countersigned by the registered professional.
* Report patient progress, status, changes or concerns (e.g., skin integrity, weight loss) to the registered professional or senior team member.
* Forge good communication with patients, service users, families, carers, colleagues and other professionals, acting as an advocate for the patient and respecting their wishes, dignity and beliefs.

South Tees Hospitals NHS Foundation Trust and North Tees & Hartlepool NHS Foundation Trust now form University Hospitals Tees and, as such, you may be required to work at any site across both Trusts.
