An opportunity has arisen for an experienced, motivated and compassionate full-time Practice Nurse to join our friendly and supportive Medical Centre comprising approximately 19,500 patients.
The successful candidate will work over 4 days per week and provide high-quality nursing care across a range of services including long-term condition management, immunisations, wound care, health promotion and routine nursing clinics. You will work both independently and as part of the wider multidisciplinary team to deliver safe, effective, patient-centered care.

Main duties of the job
Therole of the Practice Nurse is to provide comprehensive nursing care to the practice population, delivering assessment, treatment, health education and chronic disease management in accordance with practice policies, clinical protocols and professional standards.
Key Duties and Responsibilities
Clinical Duties
Provide nursing assessment, screening, treatment and health education to patients.
Deliver general nursing care independently and alongside GPs and the wider team.
Undertake wound care assessment, planning and ongoing management.
Perform ear examination and irrigation.
Undertake venepuncture where required.
Record ECGs and support interpretation.
Administer childhood and adult vaccinations in accordance with PGDs.
Administer regular injections via PSDs and agreed protocols.
Provide travel health vaccinations, information and advice, subject to training.
Manage and review patients with long-term conditions including CHD, hypertension, asthma, diabetes and COPD.
Undertake Doppler assessments for patients requiring leg ulcer diagnosis and care.
Evaluate care outcomes and adapt care plans in partnership with patients.
