An opportunity has arisen for an experienced, motivated and compassionate full-time Practice Nurse to join our friendly and supportive Medical Centre comprising approximately 19,500 patients.

The successful candidate will work over 4 days per week and provide high-quality nursing care across a range of services including long-term condition management, immunisations, wound care, health promotion and routine nursing clinics. You will work both independently and as part of the wider multidisciplinary team to deliver safe, effective, patient-centered care.

Main duties of the job

Therole of the Practice Nurse is to provide comprehensive nursing care to the practice population, delivering assessment, treatment, health education and chronic disease management in accordance with practice policies, clinical protocols and professional standards.

Key Duties and Responsibilities

Clinical Duties

Provide nursing assessment, screening, treatment and health education to patients.

Deliver general nursing care independently and alongside GPs and the wider team.

Undertake wound care assessment, planning and ongoing management.

Perform ear examination and irrigation.

Undertake venepuncture where required.

Record ECGs and support interpretation.

Administer childhood and adult vaccinations in accordance with PGDs.

Administer regular injections via PSDs and agreed protocols.

Provide travel health vaccinations, information and advice, subject to training.

Manage and review patients with long-term conditions including CHD, hypertension, asthma, diabetes and COPD.

Undertake Doppler assessments for patients requiring leg ulcer diagnosis and care.

Evaluate care outcomes and adapt care plans in partnership with patients.

Job responsibilities

Maintain treatment room stock, equipment and cold chain compliance.

Ensure emergency equipment is available and maintained.

Specimen Collection

Collect and process pathological specimens including blood samples, swabs and smears.

Administrative Responsibilities

Maintain accurate clinical records using SystmOne.

Complete all documentation relating to patient care and registration.

Generate prescriptions in line with protocols.

Support telephone calls, appointment systems and patient queries.

Assist patients with information regarding the practice complaints procedure.

Audit, Research and Development

Participate in audit, research and service improvement projects as required.

Contribute to the development of nursing services within the practice.

Attend and participate in practice meetings.

Professional Responsibilities

Maintain NMC registration and meet revalidation requirements.

Participate in appraisal, clinical supervision and ongoing professional development.

Take responsibility for own learning and development.


Person Specification


Qualifications

* NMC Registered Nurse
* Evidence of CPD
* Experience working in a primary care or community nursing setting.
* Excellent clinical and communication skills.
* Ability to work independently and as part of a multidisciplinary team.
* Competence using clinical IT systems e.g. SystmOne or similar.
* Understanding of confidentiality, safeguarding and infection control.
* Patient-focused with a caring and professional approach.
* Well organised with the ability to prioritise workload.
* Flexible, reliable and willing to develop new skills.
* GCSE x 5 to Grade 4/C or equivalent, including English and Maths.
* Verbal and written fluency in English.
* Experience of audit or service development in primary care.


Experience

* Registered Nurse NMC
* Evidence of continuing professional development.
* Experience working in a primary care or community nursing setting.
* Experience of providing general nursing care and vaccinations.
* Experience managing patients with long-term conditions.
* Excellent clinical and communication skills.
* Ability to work independently and as part of a multidisciplinary team.
* Competence using clinical IT systems e.g. SystmOne or similar.
* Understanding of confidentiality, safeguarding and infection control.
