Job Summary
We are looking for an enthusiastic part‑time practice nurse to join our friendly team in Swadlincote Surgery. The post holder will provide assessment, treatment, screening, health education and advice to patients and support the practice in chronic disease management, vaccination, health promotion and other key clinical duties.
Key Responsibilities
* Support the Infection Prevention Control lead and implement IPC policy, audits and training.
* Develop and deliver care management programmes for frail and housebound patients.
* Assess the needs of patients, deliver evidence‑based care and manage chronic disease treatment plans.
* Provide routine nursing care and manage acute and chronic conditions, including prescribing within scope.
* Carry out home visits, pathology requests and travel medicine services.
* Co‑ordinate cervical cytology, wound care, vaccination programmes and health promotion.
* Support emergency care and safeguard patients, liaising with external services.
* Maintain accurate clinical records, use read codes, and manage chronic disease registers.
Qualifications
* Registered Nurse (Nursing and Midwifery Council).
Desirable Qualifications
* Mentor or teaching qualification.
* Postgraduate diploma or degree in Chronic Disease Management.
* Nurse prescriber.
* Family planning qualification.
* Travel medicine knowledge.
Experience
* Experience of chronic disease management.
* Experience of working autonomously.
* Experience of infection prevention and control measures.
* Experience in a primary care environment (desirable).
* Experience as a practice nurse or community nurse (desirable).
* Experience with elderly and housebound patients (desirable).

Hours and Contract
Hours per week: 24–30, Monday, Tuesday and Friday. Contract: Permanent, Part‑time.
