Randstad are recruiting for a Qualified Social Worker to join Halton Council's Hospital Discharge Team. This is a full time role, offering an amazing pay rate of up to £35 per hour DOE.

Responsibilities:
8. The team supports the wards to facilitate safe and timely discharges of patients from the hospital, working within the Discharge to Assess model.
9. To achieve this they use a wide variety of personalised support, assist people to promote their independence, support people with complex needs and often changed situations to ensure they have the maximum level of choice and control available to them, promoting the principles of The Care Act and Mental Capacity Act.
10. The role is fast paced, working with adults in the hospital to plan / coordinate a safe and timely discharge OR working with adults in a community setting who are needing an urgent assessment to reduce the risk of being admitted.
11. Be part of a Duty rota.
Requirements:
12. Social Work England registered
13. Enhanced DBS
14. Social Work Degree or equivalent

15. Adults
