YorkshireCoast and Wolds Primary Care Network is made up of four GP practices The ParkSurgery, Driffield, The Medical Centre, Driffield, Eastgate Medical Group,Hornsea and Leven and Beeford Medical Practice.

The PCN provides a singlepoint of contact for other organisations to engage with primary care in ourdesignated area, thus further reducing the burden upon individual practices. Atthe same time we are available to provide additional advocacy for our localpopulation.
Job responsibilities

Accept referrals and assess how far a patientshealth and wellbeing needs can be met by services and other opportunities inthe community.
Manage and prioritiseyour caseload reflecting your patients needs.
Work with patientsidentified through the Integrated Neighbourhood Team meetings
Together produce asimple personalised care and support plan to address their health and wellbeingneeds by introducing or reconnecting people to community groups and statutoryservices, including weight management support, smoking cessation, taking upemployment, training, welfare support - personal health budget - and sign postingwhere it matters to the person.
Following a patientsjourney, evaluate how far the actions in the care and support plan are meetingtheir health and wellbeing needs.
Where required referpatients back to other health professionals within the PCN.
For your own personaland professional development, you will be expected to participate in monthlysupervision with a health professional.
Discuss concerns witha relevant GP to ensure safeguarding procedures are followed appropriately.
Work collaborativelywith local Voluntary, Community and Social Enterprise or VCSE organisations andcommunity groups to receive social prescribing referrals.
Support the VCSEorganisations and community groups to become more sustainable, sharingintelligence regarding gaps or issues identified in local provision.
Ensure all VCSEorganisations have appropriate safeguarding procedures in place and are genuineopportunities for patients to develop a sense of belonging, build knowledge, skills,and confidence.
Sharing the knowledgeand promoting the benefits with colleagues in the PCN on other servicesavailable in the local community and how these can be accessed.
Promote the social prescribingservices you offer to colleagues within your PCN and the community. Be visible.
Gather data to buildan evidence base about the benefits of social prescribing.
Ensure data iscaptured in the clinical systems and SNOMED codes are utilised.
Attends the peer support networks run by NHS Englandand NHS Improvement at ICS level
Managementof workload and personal development
Act on own initiative in prioritising and managingworkload, in respect of clinical, managerial, and administrative areas, asnecessary.
Maintain own personal and professional development asagreed at your annual review.

Qualifications
* Has undertaken the NHS England and NHS Improvement online learning programme
* Enrolled/in progress/qualified from appropriate training as set out by the Personalised Care Institute
Experience
* To understand and in practice apply Caldicott Principles, Data Protection Act 2018 and appropriately protect and individuals personal data, disclosing where appropriate only
* Demonstrable commitment to CPD
