This is an exciting opportunity for an enthusiastic, inspiring and reliable Community Health Care Assistant who is willing to support and deliver a range of high quality nursing and phlebotomy services to patients in their own homes who are registered with GP Practices within Chester South. Travel is an integral part of this post. The role is permanent with no hybrid working available. Full time – 37.5 hours per week. Site: The Lache Health Centre, Chester. Salary: £24,937 - £26,598 per annum. Closing date: 13/10/2025 23:59.
Responsibilities
* Deliver nursing services using a person-centred approach to patients in their own homes or in long-term care settings, aiming to avoid unnecessary hospital admissions.
* Work within an integrated team to provide high quality care and support to registered patients.
* Travel to patient visits; access to transport for work purposes is essential.
Qualifications and Skills
* Essential: appropriate related qualification or willingness to complete an apprenticeship relevant to the Clinical Support Worker role; completion or progression toward an apprenticeship with qualification at level 3 or above in a relevant healthcare subject; ability to demonstrate a range of work procedures and knowledge; completion of or commitment to complete the Care Certificate (CWP In-house 2-day training to be completed within 3 months of start date).
* Demonstrated understanding of confidentiality, empathy and sensitivity.
* Desirable: basic wound management; basic risk assessments and record keeping; basic clinical skills including venepuncture, skin care, wound care; experience in community care settings including personal care.
Experience
* Essential: ability to provide quality care in line with Trust policies and procedures; experience working collaboratively with colleagues and wider teams; supporting others in care settings (professional or lived experience).
* Desirable: experience with basic clinical skills and working in community care settings.
