Overview
Our Assistant Care Managers have a supportive working environment and planned inductions with help from across the team. There are 25 members in the Worthing Hospital Social Work Team, so whether it is in person, on the phone or in the chat group there is always someone on hand to help. You will be working across the Acute ward, A&E, Community Hospitals, and assessing people in their own homes; providing advice and guidance to those people who access or need the service.
As well as assessing you will be completing plans, implementing and monitoring care packages. Although there is a focus on short term support you will also be carrying out reviews for long term support. You will need to use your initiative and demonstrate personal resilience as you could have exposure to challenging behaviours from people who access or need the service. An important part of the role is accurate records of all the work you undertake, and producing reports and correspondence for a variety of audiences including meetings, conferences, court, panels and other agencies. You will receive regular, supportive and reflective supervision from senior social work colleagues, and you will also need to use your own initiative where appropriate. There would be some requirement to work flexibly, out of office hours to assist customers, carers and families and you will be required to travel throughout the county including locations that are not always easily accessible by public transport.

* Maintaining accurate records and producing reports and correspondence for a variety of audiences including meetings, conferences, court, panels and other agencies.
* Working flexibly and traveling throughout the county to assist customers, carers and families.
* Collaborating with colleagues across a multi-disciplinary and multi-agency environment to deliver services.
Qualifications and Requirements
* Ability to analyse and interpret information and prioritise needs to make recommendations/decisions with positive outcomes.
* Ability to work independently and as part of a team, under pressure and to tight deadlines.
* Ability to work in a multi-agency environment to deliver a service to those who access or need the service.
* Ability to communicate complicated or sensitive information to a range of audiences and reflect service objectives and standards.
* Sound and accurate IT and keyboard skills.
* Ability to recognise the limits of own accountability and responsibility, and to seek appropriate support, advice and supervision.
* GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
* NVQ Level 3 in Care (Desirable).
* Experience of providing support/working with people in a Social Care environment.
* Experience of forming effective working relationships within a multi-agency setting.
* An awareness of the policies, practices and procedures relevant to Social Work; experience of the assessment and review process of care packages.
* Experience of applying relevant legislation, policies and procedures to individual pieces of work.
