Job Summary
We are looking to recruit an experienced band 7 Occupational Therapist, Social Worker or Nurse to serve as a Senior Case Worker within the Adolescent Outreach Team (AOT). The role involves care coordination, assessment of an agreed caseload of AOT clients, and expert consultation to the network and referrers.
Responsibilities
* Provide care coordination and assessment for a caseload of AOT clients.
* Offer expert consultation to the network and referrers.
* Deliver coherent care through individual, family, group work and telephone support.
* Support the development of two Band 6 roles and inspire future senior clinicians.
* Participate in service development to meet the needs of young people and their parents/carers.
Collaboration
The AOT works closely with local inpatient services (Aquarius), SLP inpatient units (Snowsfields and BAU), Tier 3 community CAMHS, CAMHS Emergency Care Team (CECS), CAMHS Crisis Line and CAMHS Bed Management.
