This is an exciting opportunity for experienced Mental Health Nurses to join Nottinghamshire Healthcare NHS Foundation Trust. We have a vacancy within the Broxtowe and Hucknall Local Mental Health Team.
We will provide the successful candidate the opportunity to develop skills needed to work within a community setting, providing care to an adult population across the Nottingham County South area, open to secondary care services, within SMI pathways (mood affective, complex psychosis, community rehabilitation and non psychosis).
You will have opportunity to access training, and to develop skills in working within a specific pathway. We offer a very supportive team environment, with access to regular clinical, managerial and peer supervision
The post holder will be expected to hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
You will complete both triage assessments and more detailed initial assessments of new referrals. You will also be expected to provide support via the teams duty system, either from telephone support or face to face.
This is a leadership role and you will be expected to develop specialist skills in working with patients in at least one pathway within the team.
