Caring Forever is expanding it's Lowestoft team of Community Coordinators.

Our teams have been supporting individuals in their own home for more than 10 years. Their experience and skillset ensures Caring Forever provides personalised and adaptable care, enabling individuals to remain home comfortably, safely, and longer.

As a Community Coordinator, your duties will include:

* Working alongside the Care Manager to support them in the creation and ongoing management of care plans
* Providing care in the community (direct care)
* Conducting assessments and reviews in peoples homes
* Maintaining and improving well being
* Managing and updating assigned care plans
* Organising appointments, medication collections and general supporting tasks
* Liaising with local authorities and multi-disciplinary teams (MDT)
* Completing advanced training in specialised care areas
* Attending meetings with Care Manager and other Community Coordinator's

This role comprises of work in the community (direct care) in addition to office-based administrative duties. Community and administrative hours may be proportionate to the agreed contracted hours.

The Community Coordinator is a vital role that ensures key information, characteristics, needs, and overall support are clearly identified and maintained throughout the care & support plan of an individual. Being both a community and office-based role, the Community Coordinator is able to proactively adjust the care & support an individual receives through a combination of direct care, assessment, communicating with community teams and support networks.

The successful candidate will:

* Hold an NVQ level 2 or higher in health & social care
* Be experienced in care
* Be communicative
* Offer support, dignity and respect to others
* Be passionate at helping others and empathetic
* Have good computer skills and be proficient with Microsoft Windows & Microsoft 365 software

Candidates must have availability to work every other weekend (Saturday & Sunday). You must also have the right to work in the UK and hold a valid UK full driving license, with access to their own vehicle.
