Fairfield General Hospital is recruiting to a new and developing In-Patient Discharge Unit. We are a 24 bedded discharge unit, with a seated area for discharge lounge patients on the day. We are dedicated to getting people home by focusing on preventing deconditioning and reducing the number of days patients are kept away from home. We provide individualised discharge planning. Our ward ethos is to get people out of bed, washed, dressed and ready for the day. We engage in ward-based activities, such as arts & crafts, playing cards, painting nails, taking patients for a walk, and promoting independence as much as possible. This is a team-based approach and requires excellent communication skills, as well as the ability to engage with people from all walks of life and the multi-disciplinary team. The post holder will work as part of a team to provide a safe, effective, efficient care environment for patients, relatives, and carers. They will be responsible for the assessment, planning, implementation, and evaluation of programmes of care under indirect supervision from senior staff. They are expected to be able to demonstrate procedures and supervise unqualified staff, and work without direct supervision.
