Job Purpose
We are looking for an experienced and motivated Advanced Clinical Practitioner to join the Lewisham Community Mental Health Team. The role involves independent and team working in both clinical and managerial capacities.
The candidate should be confident in community care delivery, including medication management and physical health assessments. Excellent organizational skills, initiative, adaptability, good communication, and teamwork are essential.
The role includes leadership and clinical contributions across pathways such as screening, assessment, psychoeducation, medication management, behavioral management, care planning, risk assessment, and transition to adult services. The post holder may undertake independent prescribing and must demonstrate complex clinical reasoning and risk assessment skills to guide interventions, prevent deterioration, and avoid unnecessary A&E presentations.
