Job Title: HRV CVS Social Prescribing Link worker (Pain Management)
Based at: HRV CVS, Suite 5, The Chambers, Town Hall Square, Gt Harwood, BB6 7DD
working primarily but not exclusively with individuals living in Hyndburn
Co-Location: The post holder will spend a percentage of their time within the CVS base, and the remaining time out in the community.
Responsible to: Chief Officer
Job Purpose:
1. Managing a caseload of clients referred through the Pain Management Project
2. Supporting all aspects of HRV CVS Social Prescribing work
3. Timely reporting on the client's journey through utilising the Elemental database and different referral pathways
4. Maintaining positive relationships with funders, partner organisations and any other voluntary, community or faith groups
5. Working alongside the other projects within HRV CVS including all aspects of volunteering
6. Any other duties relevant to the post.
Key Relationships: The clients, VCF sector organisations, GP practices and the public statutory health and social care providers, the primary care network and the CVS team including volunteers.
Salary: £13.07 per hour gross
Post Status: One-year part time fixed term initially, continuation subject to
funding
Hours: 18.75 hours per week
