Classification: Registered Nurse Level 2, Health Professional Level 3 or Administrative Service Officer Grade 6 (PN 33307)
Salary RN2: $ 112,841 - $ 119,223 (Plus 12.5%25 Super)
Salary HP3: $ 111,639 - $ 120,057 (Plus 12.5%25 Super)
Salary ASO6: $ 102,657 - $ 116,592 (Plus 12.5%25 Super)
Location: The Canberra Hospital, ACT

About the Role:

Advance Care Planning plays a vital role in supporting people to make informed and meaningful decisions about their future health care. In this role, you will help individuals identify and communicate their values, preferences and goals, ensuring their voice is respected even when they are unable to express it themselves. Your work will make a meaningful difference to patients, families and clinical teams across the ACT.

You will guide sensitive conversations, assist with completing advance care planning documentation, and provide clear information to help people understand their choices. Your support ensures treating teams have the insight they need to make timely, patient centred decisions in collaboration with nominated attorneys during urgent or complex situations.

As part of the Advance Care Planning Team, you will contribute to a proactive, territory wide approach to strengthening Advance Care Planning. This includes delivering training to staff and community groups, supporting quality improvement activities, and working closely with internal and external stakeholders to enhance consistency and service outcomes across the ACT.

This is a multi-classified role with the potential for appointment at the RN2, HP3 or ASO6 level. Applicants are encouraged to indicate their preferred level and may apply across classifications.

Undergo a pre-employment National Police Check.

Prior to commencing this role, a current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required. (for Health Professionals and Administration Officers).

Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.


•

Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals. (for Health Professionals only).
