Health & Care Integration Programme Lead - 12 month fixed term contract

Are you passionate about improving health outcomes for the population and reducing inequalities? Do you have the vision and skills to help integrate health and care services and realise a shift to prevention?

Sutton’s Public Health team is seeking to recruit a Health & Care Integration Programme Lead on a 12 months’ fixed term basis. This is a joint appointment between the Council and NHS.

Health & Care Integration Programme Leads provide high-level programme management leadership to deliver our integration agenda across health and care. They oversee a range of programme areas which contribute to the delivery of the Sutton Health & Care plan, and will be jointly accountable to the Council and NHS Sutton Place (part of NHS South West London ICB).

Integration between health and care is the direction for ensuring the health and wellbeing of our residents. Partnership working between council, NHS and beyond is well-established in Sutton and this role has the opportunity to implement and develop these existing relationships further.

During this 12th month fixed term contract you will report to the Deputy Director - Sutton Health & Care Executive.

Knowledge and Experience

Essential

1. Experience (≥ 3 years) of working within an NHS, local authority or other relevant setting in a public health role.
2. Experience of undertaking evaluations to effectively measure the outcome and benefit of interventions.
3. Experience of using project management skills to take a piece of work through the project management cycle from a vision to an outcome.
4. Experience of working proactively and independently with limited supervision.
5. Experience in managing internal and external stakeholders at senior levels and facilitating/leading meetings.
6. Experience of using evidence and intelligence to identify, propose and implement service improvements and/or financial efficiencies.

Desirable

7. Clinical experience.
8. Experience of change management and/or working with groups to facilitate buy-in and ownership.
9. Experience of strong relationship building and influencing with senior stakeholders.
10. Experience of working with Neighbourhoods or Integrated Neighbourhood Teams

Skills and Abilities

Essential

11. Knowledge of best practice health & care integration in local contexts.
12. Awareness of the political, economic, financial and organisational complexities of local government and the NHS.
13. Ability to quickly assimilate a wide range of information.
14. Ability to undertake robust and accurate quantitative analysis, to analyse the results and to use the findings to develop coherent arguments and proposals.
15. Ability to think independently and creatively. Able to argue for solutions on the basis of evidence and to scrutinise and challenge accepted ideas.
16. Ability to provide and receive highly complex, sensitive, or contentious information, and manage difficult conversations with tact, diplomacy and political awareness.
17. High standard of communication skills, demonstrated by the ability to present complex oral and written information, in a variety of formats to a variety of audiences.
18. High standard of report writing skills, demonstrated by the ability to write and contribute to reports of varying length and complexity, for a variety of audiences, in a format where implications of the report are fully understood and inform strategic decision-making.
19. Ability to work in partnership with other organisations and develop trusting relationships, consolidating different perspectives, identifying areas of mutual interest and agreeing joint objectives.
20. Excellent time management and prioritisation skills, demonstrated by the ability to use own initiative to manage a number of projects simultaneously, ensuring planning, organising and prioritising work load to meet project deadlines and deliver agreed objectives within budget.
21. Ability to anticipate barriers to delivery and identify innovative ways for overcoming these.
22. Ability to work in an uncertain and evolving environment.
23. Strong IT skills.
24. Commitment to excellent public services.

Desirable

27. Knowledge of the public health system in London.
28. Knowledge of the local government improvement agenda including Council performance management functions and improvement methodologies.
29. Knowledge of the NHS 10 year plan and its direction towards neighbourhood working

Qualifications

Essential

30. High level qualification in Public Health or equivalent experience.

Desirable

31. Relevant project management qualification/accreditation.
32. Clinical qualification.

About Us

About Us

The Health & Care Integration team is a busy and dynamic team. It reports both into Public Health (the Council) and NHS Sutton Place. We work with partners from across the health and care system leading the development and monitoring implementation of the Sutton Health & Care Plan (and Neighbourhood Plan) on behalf of the Sutton Alliance.

The Public Health team produce a range of strategic documents, (including the Joint Strategic Needs Assessment (JSNA), Pharmaceutical Needs Assessment (PNA) and the Annual Public Health report) and provide support and input to teams commissioning PH services, including NHS Health Checks and services to promote healthy lifestyles.

The Public Health team is part of the wider Public Health and Wellbeing directorate which delivers:

36. Children’s Community Health Services: Health Visiting, School Nursing, Child Safeguarding Nurses, Early Intervention (Children’s Centres, Pre-Schools, Play Service)
37. Public Health Commissioned Services e.g. Substance Misuse, Sexual Health, NHS Health Checks Quality Assurance Health Service Commissioning
38. Libraries, Culture & Leisure, Transport for Older People, Meals on Wheels, Sutton College, the Social Inclusion Centre and the Life Centre
39. Sutton Registration Service
40. Accountability for Voluntary and Community Services


On the NHS side, you will report into NHS Sutton Place. The Place team supports the design, planning and coordination of health and care services at the Sutton level, covering community and primary care services.
