Pathway Coordinators are anintegral part of the Outpatient team, looking after patients from the pointof referral to discharge, ensuring that the process is both efficient andeffective. This is achieved byaccurately processing and actively monitoring the flow of informationthroughout the patient pathway.

The role would suit someone who isgood at multitasking, problem-solving with an empathetic nature, who has apassion for helping patients.

The Pathways team is currentlymade up of 8 staff members based in the Outpatients department at DorkingCommunity Hospital. As we are seeing the number of patients grow, we arelooking to expand our team.

We would be interested in people who could offerbetween 30 to 37.5 hrs a week.


Main duties of the job

Pathwaycoordinators are responsible for ensuring that the patient journey is bothefficient and effective. This involveschasing reports, results, and other documentation with GPs, hospitals, and third-partyproviders to ensure these are in place prior to appointments. When processingreports, the pathways team will highlight any urgent findings to our cliniciansto expedite patient care where required. The ideal candidate would therefore besomeone who can remain calm and maintain a high level of attention todetail.

The successful applicant will be able to help manage the workloadefficiently and effectively on a daily basis to facilitate the patient's carebeing as smooth and efficient as possible.

We will provide the training necessary to use our in-house IT systems;however, a good working knowledge of Microsoft Office would be beneficial. You will be required to maintain patientconfidentiality at all times.

Job responsibilities

JOBSUMMARY

The role (and team) is an integral part of thepatient pathway (referral to discharge), ensuring that the process is bothefficient and effective. This is achieved by accurately processing and activelymonitoring the flow of information throughout the patient pathway.

ThePathways team looks through each patients referral carefully to make sure theclinician has all the relevant information about the patients conditionavailable to them on our electronic record system. This means the patients get the most out oftheir time with the consultant as all relevant test results and images areavailable for review.

Wework with several NHS trusts and independent hospitals in the local area andhave our own in-house diagnostic services. A part of the Pathway role is toforward on results from tests to the patients GP surgery as well as ourconsultants.

You will need to have excellent attention to detailand use initiative and good judgement to problem solve and consider the bestcourse of action to take for the patient.

DUTIESAND RESPONSIBILITIES OF THE POST

* Prepare clinics for each clinician across allspecialities ahead of their clinics, which will include details about theirexpected appointments, routine results that have been received, pathologyresults for filing, and patient queries.
* For new patient appointments, thoroughly review thereferral information provided by the GP, picking out investigations or teststhat have already been completed and adding them to the patients electronicrecord.
* For follow-up patient appointments, review theprevious clinic letter and referrals out recommended and ensure these have beenconducted and the results are available for review.
* Process post-clinic folder documentation by distributingand actioning the contents as required.
* Shared responsibility for managing the Pathwayshared email box and the shared tasks on our patient record system, actioningthem as appropriate within a timely fashion.
* Communicating and sending the results of diagnostictests we have conducted in-house to the patient's GP within the specifiedtimeframes.
* For results we have received from externalproviders for our patients, review the results, add to the patient's record, andtake the necessary follow-up actions, removing patients from waitlists, bookingfollow-up appointments, for example.
* There may be urgent findings on reports that needactioning immediately, following processes to highlight this to the clinicianand follow their guidance on the outcome required to progress the patient'scare.
* Chase reports, results, and other documentationwith GPs, hospitals, and third-party providers, working proactively andindependently to achieve required outcomes.Manage the incoming and outgoing image requests.
* Track patient pathways from referral to discharge,highlighting and resolving queries as they are encountered.
* Maintain excellent working relationships withcolleagues, consultants, and external providers.
* Maintaining accurate patient records by updatingwait lists, updating tasks, saving delivery/read receipts, and adding resultsand notes.
* Manage multiple tasks, prioritising own workloadand working with the team to ensure work is completed within target timeframes.
* Undertakeany other reasonable duties as commensurate with the nature of this role.
* Undertakeany mandatory training as appropriate to the post and as directed.

WORKSETTING AND REVIEW

Tomanage own workload efficiently and effectively on a daily basis with a degreeof autonomy. Maintain confidentiality atall times. Make decisions pertaining tothe smooth and efficient running of the department, acting independently but,where necessary, referring problems to management or the appropriate consultantvia the quickest route possible.

CONFIDENTIALITY

Thepost holder must maintain the confidentiality of information about patients,staff, and DHC business in accordance with the Data Protection Act 1998 and theCaldicott principles.

Qualifications

* Good standard of education
* (Numerate and good standard of written and spoken English)
