Bury Town Integrated Neighbourhood Team is looking for a District Nurse to work alongside our established team of two other District Nurses. Based within an INT comprised of two Community Nurse teams, OT's and Physiotherapists with support staff, this role offers the opportunity to join one of the largest INTs in West Suffolk. You will be at the front line of healthcare, delivering medical care to patients on our caseloads in their own homes with excellent support from an Integrated Community Team. This position requires a specialised District Nursing degree following an Adult Nursing qualification. NHS work experience is essential & Community Nursing experience desirable. The role helps maintain a patient’s independence and freedom while providing exemplary medical care.
Responsibilities
* Lead and manage the District Nursing Service in this locality, providing specialist clinical and supervisory leadership to junior staff and students.
* Manage a caseload of patients with multiple and complex needs, using evidence‑based practice to assess, plan, implement, and evaluate interventions, keeping the patient at the centre.
* Provide the highest standard of clinical care by using advanced skills and expert knowledge to holistically assess needs and instigate and provide clinical treatments.
* Lead service development of integrated services in partnership with peers and colleagues.
* Use new knowledge in innovative ways, taking responsibility for developing and changing practice in complex and sometimes unpredictable environments.
* Support staff to feel confident and competent to move across organisational and professional boundaries.
* Monitor the quality of care provision, identify, and promote areas for service development.
* Build strong relationships with partner organisations, especially for patients receiving shared care, ensuring an effective flow of patient information for high‑quality care.

* Registered Nurse Level 1
* District Nurse Qualification
* Non‑medical prescribing qualification
* Preparation for Mentorship qualification
* Current NMC registration
* Evidence of continuing professional development

* Tissue viability or leg ulcer management training
