Job Summary:
The Team Leader is an essential part of the support team, responsible for addressing the practical, physical, and emotional needs of the individuals we support within the service.
Using a key worker system, they will design individual support programs and coordinate their delivery by Support Workers and themselves.
Key Accountabilities:
* Plan, coordinate, and facilitate the safe transition of new individuals into the service and community where necessary.
* Communicate with families of the supported individuals and relevant multi-disciplinary teams, community groups, and voluntary organizations to promote their needs.
* Develop and implement individual and group support programs to enhance independence and capabilities.
* Conduct ongoing assessments and reviews using appropriate models.
* Support the physical, emotional, and personal needs of the individuals, ensuring respect, privacy, dignity, and independence.
* Manage medication according to Praxis Care’s policies and legal requirements.
* Provide leadership, supervision, and management to Support Workers, including monitoring attendance and participating in HR procedures.
* Engage in learning and development, identifying growth opportunities for self and team.
* Maintain accurate records and produce reports as per policies and legal standards.
* Assist in ensuring regulatory compliance with relevant bodies.
Requirements of the Role:
* Participate in appraisals and staff reviews.

* Perform duties in line with organizational policies and regulations.
* Be flexible with working hours to meet service needs.
* For sleep-in duties, an additional payment of £45.00 per night applies.
The duties are not exhaustive and may evolve with organizational needs.
Education, Experience & Qualifications:
* At least 2 years’ experience in a health and social care setting, with a commitment to professional development, OR
* Qualifications in Social Work, Allied Health Professions, or a Level 5 Diploma in Leadership and Management for Health and Social Care.
* Alternatively, 1 year paid experience in HSC with a Psychology Degree and commitment to qualification.
Candidates should be prepared to complete Level 2 Safe and Effective Care Practice Diploma or be registered with NISCC/NMC or an Allied Health Professional.
Must have a full valid driving license and access to a vehicle, or demonstrate how mobility requirements are met if disabled.
Skills include strong communication, report writing, assessment, evaluation, problem-solving, and organizational abilities.
