Role Purpose
Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co‑occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated.
Adferiad Recovery harnesses the talents and experience of renowned Welsh charities CAIS, Hafal and WCADA. Our combined expertise in substance use, mental health, housing, criminal justice, and employment support enables Adferiad Recovery to meet the needs of our most vulnerable with a single, unified and comprehensive approach.
The Denbighshire Collaborative is a trusted and experienced partnership jointly delivered by Adferiad and Stori, who together have over 20 years of experience working in Denbighshire and over 40 years across Wales.
The Denbighshire Collaborative delivers a new, holistic, housing‑related support project designed to support people living in Denbighshire to sustain their accommodation and prevent homelessness. The support provided will be flexible and person‑centred, supporting people to maintain sustainable housing by addressing any mental health, substance use or other problems they may face, helping to improve their health and well‑being.
Key Responsibilities

* Assist individuals in maintaining sustainable housing by addressing mental health, substance use, or other issues impacting their living situation.
* Support the improvement of health and well‑being through tailored interventions and guidance.

Diben y Rôl: Mae Cydweithfa Sir Ddinbych yn bartneriaeth ddibynadwy a phrofiadol a gyflwynir ar gyfer Adferiad a Stori rhwng yn bros rau dros 20 mlynedd o brofiad o weithio yn Sir Ddinbych a thros 40 mlynedd ledled Cymru.
Mae Cydweithfa Sir Ddinbych yn cyflwyno prosiect cymorth newydd, cyfannol, sy'n gysylltiedig â thai, wedi'i gynllunio i gefnogi pobl sy'n byw yn Sir Ddinbych i gynnal eu llety ac atal digartrefedd.
