Mid Devon Healthcare is the Primary Care Network supporting Bow Medical Practice, Redlands Primary Care and Wallingbrook Health Group.
In 2019 Primary Care Networks were set up by the NHS to help support General Practice. It enabled the practices to recruit different skills to support the existing GPs and Nursing teams in practice. The PCN clinical team includes Pharmacists, Health Coaches, Social Prescribers, Frailty Co-Ordinator, Mental Health Practitioner, Occupational Therapist.
Main duties of the job
The post holder will work within their clinical competencies as part ofa multi-disciplinary team to provide expertise in clinical medicinesmanagement, structured medication reviews, manage long term conditions,management of medicines on transfer of care and systems for safer prescribingand support acute prescription request, while addressing both the public healthand social care needs of patients within the GP Practices within thePCN.
The post holder will perform face to face and telephone medicationreview of patients with polypharmacy especially for older people, and thosewith multiple comorbidities.
The post holder will provide quality improvement andclinical audit and well as managing some aspects of the Quality and OutcomesFramework. This role is pivotal to improving the quality of care andoperational efficiencies so requires motivation and passion to deliver anexcellent service within General Practice.The post holder will be supported to develop their role to become a non-medicalprescriber, if that qualification is not already held.

Job responsibilities
Undertake clinical medication reviews with patients and producerecommendations for Nurses and/or GP on prescribing and monitoring.See (where appropriate) patients with single or multiple medical problems wheremedicine optimisation is requiredReview the on-going need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines. Make appropriate recommendations to GPs formedicine improvement.Risk stratification: Identification of cohorts of patients at high risk of harmfrom medicines through pre-prepared practice computer searches. This mightinclude risks that are patient related, medicine related, or both.Unplanned hospital admissions. Review the use of medicines most associated withunplanned hospital admissions and readmissions through audit and individualpatient reviews. Put in place changes to reduce the prescribing of thesemedicines to high-risk patient groups.Provide medicines support for patients with questions, queries and concernsabout their medicines.
Management of medicines at the transfer of care. To reconcile medicinesfollowing discharge from hospitals, intermediate care and into Care Homes,including identifying and rectifying unexplained changes and working withpatients and Community Pharmacists to ensure patients receive the medicinesthey need post discharge. Set up and manage systems to ensure continuity ofmedicines supply to high-risk groups of patients (e.g. those with medicinecompliance aids)
Medicine information to Practice staff and patients. Answer relevantmedicine-related enquiries from GPs, the wider MDT team, other network staffand Healthcare Teams.
Support andwork alongside the Lead PCN Clinical Pharmacist, Pharmacy Technician, Prescription Teams and Dispensary Teams supervising and advising when appropriate. Work alongside the extended PCNPharmacy Team to deliver the PCN shared agenda.
PROMOTE PRESCRIBING QUALITY AND SAFETY INCLUDING:
Support repeat prescribing policy within each Practice.
Manage the repeat prescribing reauthorisation process by reviewingpatient requests for repeat prescriptions and reviewing medicines reachingreview dates and flagging up those needing a review. Ensure patients haveappropriate monitoring tests in place when required.
Analyse, interpretand present medicines data to highlight issues and risks to support decisionmaking. Undertake clinical audits of prescribing in areas directed by the GPs, feedbackthe results and implement changes in conjunction with the relevant practiceteam. Implement changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.
Implementation of local and national guidelines and formularyrecommendations. Monitor Practice prescribing against the local healtheconomy's RAG list and make recommendations to GPs for medicines that should beprescribed by Hospital Doctors (red drugs) or subject to shared care (amberdrugs).Liaise with colleagues including ICB Pharmacists and Pharmacy Technicians Headsof Medicines Management/Optimisation to benefit from peer support. Foster andmaintain strong links with all services across the PCN and neighbouringnetworks. Explore the potential for collaborative working and takeopportunities to initiate and sustain such relationships. Liaise with otherstakeholders as needed for the collective benefit of patients, including butnot limited to: Patients and their representatives GP, Nurses and otherPractice staff Social Prescribers, First Contact Physiotherapists, Physicians,Associates and Paramedics and hospital staff.PROFESSIONAL DEVELOPMENT
Work with your line Manager to undertake continual personal andprofessional development, attend regular CPD sessions with the extended MDTteams at both practices. AttendClinical, Prescription and Dispensary team meetings. Taking an active part in reviewing anddeveloping the role and responsibilities. Adhere to organisational policies andprocedures, including confidentiality, safeguarding, lone working, informationgovernance, and health and safety. Work with your line Manager to accessregular clinical supervision, to enable you to deal effectively with thedifficult issues that people present. Review yearly progress and develop clearplans to achieve results within priorities set by others.

* Maintain registration with the General Pharmaceutical Council
* Relevant undergraduate degree in pharmacy (BPharm, MPharm or equivalent)
* Minimum of 2 years post registration experience
