This is an exciting opportunity for a motivated and experienced Social Work Senior Practitioner to join our Looked After Children Service in Shrewsbury. Responsibilities
* Undertake ongoing assessments and interventions with children and families to ensure the right services are accessed at the right time.
* Progress children’s plans, ensuring the voice of the child is integral to case planning.
* Visit and work with families as needed, assessing and reassessing the needs of children looked after by the local authority.
* Act as a Case Manager and co-ordinate appropriate review meetings.
* Serve as a Key Worker where a Children Looked After Plan is in operation.
* Provide and contribute to Child & Family Assessments.
* Develop methods for identifying and assessing children in need as per the Children’s Services Plan.
* Offer information and advice to assist children and families in resolving issues.
* Liaise with other agencies on behalf of service users, coordinating resources and arranging assistance.
* Carry out duties as authorised by senior management.
* Present evidence and reports to courts as required.
* Participate in the team’s Duty system as needed.
* Maintain computerised records and documentation according to approved policies and procedures.
* Collaborate with parents, guardians, carers, and other professionals regarding Children Looked After and maintain records as required.
