The Exeter (West) and Exeter (South) Integrated Health and Social Care teams is looking to appoint 2 full time (37 hours per week) Social Workers.
The teams are made up of a range of health and social care professionals including therapists, nursing staff, community matrons and social care professionals. We also work alongside and in partnership with our GP services. We are committed to a person-centred culture, putting people at the forefront of what we do. We are a supportive team who adopt a forward-thinking, proactive approach to continuously shaping and delivering innovative professional practice.
As a service we support a wide range of service user groups including but not limited to;
1. Older People
• People with Primary Mental Health Needs
• People with Learning disabilities
• People affected by Substance Misuse
• People receiving End of Life Care
• Younger Adults
2. People leaving hospital
• Carers
As a Social Worker working with us you will be supported to manage a complex caseload. This includes investigating safeguarding concerns, conducting statutory assessments, court work, commissioning support, and developing support plans.
You will collaborate with partner agencies, carers, and families to build networks of support that can help people maintain their independence.
As a service, we are committed to working with individuals to build on their strengths, enabling them to live full lives within the community as independently as possible.
You will need a good understanding of current & relevant legislation such as the Care Act (2014), the Mental Capacity Act (2005), the Equality Act (2010) & Human Rights Act (1998)
We are committed to continued professional development. You will receive regular supervision.
Hybrid working arrangements are applicable to the role, this includes elements of office-based working, client visits and working from home.
