Senior Integrated Transfer of Care Co-ordinator

This is a new and exciting post! East Coast Community Healthcare (ECCH) is proud to partner with the Norfolk and Waveney Integrated Care Board to introduce a Senior Transfer of Care Coordinator role within our Transfer of Care team, supporting local integrated Care Systems.

Are you caring and compassionate about patient care and making a difference to wider health and care systems? As the senior coordinator, you will be responsible for the smooth day-to-day operation of the team, ensuring effectiveness and efficiency.

Main duties of the job

The post holder will work closely with system partners from the ICB, ECCH, JPUH, SCC, and NCC, representing their views and work collaboratively on behalf of the GYW area.

They will support safe patient transfers in the Great Yarmouth and Waveney Locality, promoting community patient flow using the Home First ethos, and managing East place rehab beds. Communication and liaison with multiple system partners, patients, and families are crucial.

The role includes supporting performance, development, and quality within the Transfer of Care Team and the wider East Place system to meet UEC priorities.

You will lead data collection projects, ensuring accurate and timely data to monitor trends, and communicate findings through reports and briefings. Strong IT skills are essential.

You will lead relevant working groups and initiatives, providing analytical advice to strategic leads.

Job responsibilities

Project Management and Operational Responsibilities

* Lead small-scale projects and initiatives.
* Support data collection, analysis, and reporting to inform decision-making.
* Participate in internal and external working groups, providing advice and information.
* Develop and review patient transfer and discharge processes, proposing improvements.
* Implement services aligned with East Place priorities.
* Develop and monitor systems to evaluate service quality and meet targets.

Staff Management

* Line management of the Transfer of Care Coordinators, including supervision and development plans.
* Plan staffing rotas and annual leave.
* Support training, induction, and recruitment of team members.
* Deputise for the Transfer of Care Manager as needed.
* Provide operational leadership to system partner teams and oversee daily tasks and professional supervision.
* Collaborate with health and social care colleagues, escalating delays when necessary.
* Participate in daily system meetings to support patient flow and discharge planning.

Professional Development

* Contribute to developing key performance indicators and support service improvement initiatives.
* Conduct research to support local and national guidance.
* Analyze data to identify trends and inform service delivery.
* Communicate effectively with stakeholders and act as a role model for best practices.

Person Specification


Qualifications

* Professional registration or equivalent experience in health, social care, or voluntary sector.
* Leadership or management qualification/training or equivalent experience.
* Ongoing professional development.
* IT training (PRINCE2) or experience in project development/data analysis.
* Relevant accredited courses or training.


Skills and Knowledge

* Negotiation and influencing skills.
* Excellent communication with stakeholders.
* Experience in project work and data analysis.
* Problem-solving skills.
* Proficiency in IT systems (SystmOne, OPTICA, MS Office).
* Ability to travel as required.
* Ability to lead change and communicate complex information effectively.
* Understanding of discharge pathways and delay codes.

* Partnership working and multidisciplinary care coordination.
* Experience with discharge processes and patient flow management.
* Use of electronic patient records and team leadership.
