Pertemps are recruiting for experienced Adult’s Social Workers in North London.
This to work within a Supported Living Review function.
The service aims to
* Ensure the delivery of least restrictive and most cost-effective support models at a scheme level, enabling individuals to achieve their outcomes.
* Review the quality and affordability of accommodation.
* Ensure the delivery of quality care and support at a scheme level.
You will need to have experience of
* performing reviews to ensure that assessments and support plans accurately represent the support outlined by the service and meet the individuals' outcomes.
* translating review information into structured documentation to enable the calculation of proportionate support.
There is plenty of hybrid working available.
