About the Role
Liquid Personnel is recruiting qualified and experienced Adult Social Workers to join a Hospital Team in Lambeth.
Responsibilities
* Support residents admitted to hospitals within and outside the borough.
* Deliver strength-based preventative support and assess needs.
* Establish support systems and safeguard patients.
* Review care packages and consider placements.
* Refer individuals to reablement services to improve living skills and facilitate timely discharges.
* Collaborate with multidisciplinary teams, voluntary sector organisations, and other agencies.
* Apply your experience with adults with physical health disabilities and vulnerabilities.
* Maintain a strong understanding of safeguarding procedures.
Qualifications & Experience
Applicants must have Social Work England registration, a full UK driving license, a degree or equivalent in Social Work, and at least 2 years of post-qualified experience.
