We are seeking an experienced and forward looking Nurse to support our community services to further progress the neighbourhood health working in SDHC. As the Neighbourhood Health Clinical Lead, you'll provide clinical leadership to our nursing teams to drive forward joined-up, equitable care within local communities. You will work closely with Primary Care Networks (PCNs), voluntary sector partners, and residents to champion proactive, population-focused approaches, providing care closer to home.

As part of the senior leadership team this role will lead on initiatives and plans across a range of issues, particularly with respect to continuous improvement in patient care delivery pathways and clinical quality. They will provide assurance of proactive management of clinical and cost improvement plans, ensuring appropriate actions are taken to mitigate or respond to identified risks. They will be responsible for the delivery of a complex agenda which drives efficiency and quality initiatives across the partnership.

Main duties of the job

* Work in collaboration with the clinical leads for PCNs and Specialist services to provide senior nursing expertise that facilitates the effective deployment of the nursing workforce and secures high standards of nursing care and intervention.
* In conjunction with colleagues this role will support, challenge and contribute to the performance and quality review process.
* It will also provide professional advice/expertise on all matters relating to professional nursing practice and future progression of neighbourhood health service in Surrey Downs.
* Support and be the deputy for Associate director of Nursing.

Detailed Job Description And Main Responsibilities

* To provide overarching clinical support for Neighbourhood Health Teams.
* To lead on and support the development of Neighbourhood Health.
* To support teams to deliver the objectives of the Neighbourhood plan.
* To identify and analyse the weekly and monthly data and assist in development of performance dashboards and service improvement linked to the information.
* Lead on service development projects to further embed Neighbourhood Health.
* To support and deputise for the Director of Multi-Professional Leadership and the Associate Directors of Nursing within Surrey Downs Health & Care as required.
* To work collaboratively with the Operational Managers of the PCNs and Specialist Services and providing clinical guidance, expertise and supervision for clinical leadership across the PCNs and Specialist Services (as required).
* To assess and identify the needs of clients with a variety of complex social and health care needs.
* To lead and participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population.
* To promote effective teamwork within the wider Surrey Downs Health and Care, delegating activities to the wider team to the benefit of the patient and service delivery.
* As a Clinical Lead within Surrey Downs Health and Care there will be the opportunity to link in closely alongside multiple other services and organisations
* The expectations within this job description will be achieved through clinical practice, service development, education, training and research.
* To work with the Associate Director of Clinical Operations to ensure clinical safety, governance and quality.

Person specification

Qualification Essential criteria

* Current registration with NMC
* Experience and/or additional education in relevant fields to post grad diploma or equivalent level, combination of education, further training and experience
* Evidence of CPD relevant to assessment and treatment of complex patients in hospital and in the community

Experience Essential criteria

* Recent post graduate experience as a senior clinician within the community settings
* Experience working within a multi-disciplinary team
* Experience and evidence of leading and participating in service development
* Experience of supervision and teaching others
* IT skills to include, but not limited to Excel, Word, PowerPoint, internet, email, patient recording systems

* Team Leadership
* Valid drivers licence with access to use of car for work
