As a Patient Pathway Coordinator, you will play a pivotal role in ensuring the seamless management of patient pathways within our esteemed Diabetes service. You will be joining a highly efficient team dedicated to maintaining the highest standards of patient care and service delivery.
Key Responsibilities
* Be the point of contact for patients accessing elective hospital services throughout their pathway of care.
* Work under the direction of the Senior Patient Pathway Coordinator / Speciality Consultant Team and provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
* Provide a range of administrative support services to the speciality Consultant Team.
* Ensure high levels of patient care and clinician satisfaction by being an accessible, customer-focused, and knowledgeable point of contact.
* Adhere to Standard Operating Procedures (SOP) to ensure a consistent approach across the Trust, describing how the functions of this role will be carried out along with the timescales that will be adhered to.
Person Specification
Knowledge
* Computer literate, with experience of using a keyboard with a high level of accuracy.
* Knowledge of NHS admin processes
* Thorough knowledge of healthcare administrative systems and processes
* Knowledge of databases, spreadsheets i.e. PAS, Excel or similar applications
* Confidentiality and Data Protection
* Health and Safety
* Equality and diversity
* Sound office clerical background providing knowledge of relevant procedures.
* Experience of using a full range of IT/Patient data systems – as applicable to speciality
Qualifications
* Good general education with GCSE Maths and English at grade C or above.
* RSA II (or equivalent)
* ECDL
* Association of Medical Secretaries, Practice Administrator & Receptionist diploma (AMSPAR)
* NVQ in Customer Care
* Institute of Health Records & Information Management Diploma (IHRIM)
* Shorthand speeds of 80 wpm
Further Training
* On the job training
* Supervisory training
* Experience of managing health & safety issues
Experience
* Quality Customer Service
* Some experience providing required NHS knowledge and qualification equivalences if necessary
* Experience of supervising staff
Contractual
* Regular attendance and willingness to work flexible hours as necessary
